Replacing Sugar: The Healthiest Sweeteners and the Worst Ones

Cutting down on sugar is one of the most common health goals, and the shelves are full of alternatives promising to help. But not all sweeteners are created equal, and some of the "healthy" ones are less different from sugar than the marketing suggests. Here is an evidence-based look at which sweeteners are worth considering, which are best limited, and what the science actually says.

First, why limit sugar at all?

The concern is mainly about free sugars: the sugars added to food and drink, plus those naturally present in honey, syrups and fruit juice. Health bodies including the World Health Organization recommend keeping free sugars to less than 10 percent of daily energy, and ideally below 5 percent, because high intakes are linked to tooth decay, weight gain and related health problems. Importantly, this applies to most "natural" sugars too. Honey and maple syrup still count as free sugars, a point that often gets lost.

The better everyday options

If you want to sweeten food with something less refined, a few whole-food based options have small advantages, mostly because they bring a little flavour and trace nutrients rather than pure sweetness alone.

Coconut sugar has a slightly lower glycaemic index than table sugar and retains small amounts of minerals, though nutritionally it is still very close to ordinary sugar and should be used in the same modest amounts. Dates and date syrup are another useful option, since they bring fibre and a rich caramel flavour, which is why blended dates are so popular in energy balls and baking. Honey and maple syrup can add distinctive flavour, so you may use less of them, but they remain free sugars and offer no meaningful nutritional free pass.

Non-nutritive sweeteners: the sugar-free route

Sweeteners that provide little or no calories are a different category. Stevia, a plant-derived sweetener, and sugar alcohols such as erythritol and xylitol deliver sweetness with minimal effect on blood sugar, which is why they are common in low-sugar and keto products. For people managing blood glucose or cutting calories, these can be genuinely useful swaps.

A couple of caveats are worth knowing. Sugar alcohols can cause bloating or a laxative effect in larger amounts, and xylitol is highly toxic to dogs, so keep it well away from pets. It is also worth noting that the World Health Organization has advised against relying on non-sugar sweeteners for long-term weight control, since the evidence that they help is limited. They are best seen as a tool for reducing sugar, not a health food in their own right.

The ones to limit most

At the other end of the scale sit the sweeteners with the least to recommend them. High-fructose corn syrup, common in mass-produced soft drinks and processed foods, is a concentrated source of free sugars with no nutritional benefit and is strongly associated with excess calorie intake. Ordinary refined white sugar is not "toxic," but it delivers pure free sugars with nothing else, so it is the one most of us should cut back on. Agave syrup deserves a mention too: often marketed as a healthy choice, it is very high in fructose, so it is best treated like any other syrup rather than a virtuous upgrade.

The most powerful change is not swapping one sweetener for another but gradually reducing how much sweetness you use and expect. Our taste for sweetness adapts over time, and palates adjust when given the chance. Whole-food options like coconut sugar and dates can be a helpful step, and non-nutritive sweeteners have their place for specific goals, but no sweetener is a licence to over-indulge. Used thoughtfully and in moderation, though, the right swaps can make cutting back on sugar a good deal easier.
